		"I GOT SKILLZ" 3-ON-3 BASKETBALL REGISTRATION Friday, January 9, 2009 at University Area Community Center. The games begin at 6PM. Team must register by December 13, 2008 for consideration To register your team, please complete the form below. Teams are categorized by males & females 11 - 14 years old and 15 - 18 years old. After completing form, please mail payments ($25/ per team) by December 20th to GZL Educational Foundation,Inc., P.O. Box 82102, Tampa, FL 33682 Registration is NOT COMPLETE UNTIL ALL FEES ARE PAID. All player must have insurance waivers signed and returned before they are allowed to play. The coach will be the point of contact and must be at least 18 years old.
