After completing form, please mail payments ($80/golfer) before October 3rd to: GZL Educational Foundation,Inc., P.O. Box 82102, Tampa, FL 33682 Please note that registration is NOT COMPLETE UNTIL ALL FEES ARE PAID for each player. Click on the PAYMENTS link to make your payment. Payments after Oct. 3rd will be charged at $90 per player and payments on the day of the tournament will be charged at $100.00 per player.
